We are delighted to say that we have achieved the new Platinum award for carer’s support in 2018. This award is in recognition of our hard work in providing services for our patients who are Carers and to help them in the role they take on when supporting a loved one.
At Kennet and Avon Medical Partnership, we hold a Carer’s register and any patient who is a Carer can be added to this by collecting a pack from the reception team, completing the form and returning to reception, or by completing our “Register a Carer” form on our website. We will then notify Carers Support Wiltshire which is an organisation providing relevant information and advice, a regular newsletter and offers local support services.
We also hold Carers Support Clinics during the year. If you would like to attend one of these clinics please indicate this on the carer’s registration form. The appointment includes a 20 minute appointment with our Health Care Assistant and a 20 minute appointment with Carers Support Wiltshire to offer support, advice and information as required.
We are also able to offer carers flexible appointments with all our clinical staff, to suit their needs as we understand that looking after your own health can be challenging when caring for someone else, and that your time is limited.
