Be smart, do it online…

The best way to access you personal details is through you NHS online account.

If you want to have access, in real time, to a version of your medical record, please register for an NHS account if you have not already done so.

My NHS Number

Your NHS number is a 10-digit number, unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records. To find yours, use the link below.

Test Results

The time taken for results to be received varies depending on the type of test and where it is processed.

If your result shows that you need more tests or treatment, a member of the team will contact you to confirm what is required.

We will not normally contact you about your test results if they are as expected. You are welcome to contact us if you would like the result, and our team will be able to share these with you only once they have been reviewed by a clinician.

Obtaining a copy of your medical record

If you want to obtain a copy of part of or your medical record, a “Subject Access Request (SAR) needs to be made. We have a calendar month from receiving your request and the necessary permissions to respond to such requests.

Please think carefully before you request a copy of your records. Your record may be held in both paper form and electronically. We will have to copy both parts, and then a GP will have to review the entire record to check that there are no entries that contain personal data belonging to another individual. Where the GP finds such third party data, we will have to remove it from the copy that we are preparing for you.

Please help us to help you by making reasonable requests of our valuable time. In particular, do you need a copy of your entire record, or only a part of the record, for example covering a particular period? The form that you must complete to request a copy enables you to ask for just a part of your record.

Access Someone Else’s records

There are some circumstances where a parent, family member or carer, you may be able to access the medical record for someone else, to allow you to, for instance, order their repeat medication online. We call this having proxy access.

In most circumstances, where a patient is over 14, they are able to agree to give access to another person, and would do so by completing our online form.

Proxy Access

To request proxy access, please collect a proxy access form from our patient navigators at the Practice or download and print the form using link below. Please bring form and your ID to the practice to complete the process.

Alternatively follow online link to start the process – you will still need to come into the practice with your ID to complete the process.

Linked Profiles

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
